Lesson #: Infections Can Cause Damage To Heart Valves

As many of you know, there are two general types of infections: viral and bacterial.

There are two ways that bacterial infections can affect the valves of the heart.

The first is that bacteria circulating in our blood stream as a result of an infection can latch on to any of the heart valves as the bacteria within the bloodstream are pumped through the heart. The bacteria that attach themselves to the leaflets of the heart valves can replicate and either physically destroy the heart valve by forming an access or the bacteria can replicate to such a degree that they form a vegetation on the heart valve. This vegetation can get to the point where it mechanically interferes with the opening and closing of the heart valve leaflets.

Once the infection causes an access that dissolves the heart valve tissue, it is necessary to treat the access with open heart surgery and removing the infected heart valve and replacing it with an artificial heart valve.

A heart valve vegetation can sometimes be resolved with many weeks if intravenous antibiotics. Other times, the vegetation needs to be resected with open heart surgery because it is resistant to the intravenous antibiotics or is interfering with the function of the heart valve to such a degree that the valve function is so poor that it is compromising the heart function.

This is not what Mr. Paxton had.

Mr. Paxton experienced a different type of infection-related heart valve complication.

Insist On An Open Exchange

When youre considering heart surgery, nothing about the operation should be a mystery. The biggest fear patients have is of the unknown, says Suzanne Fredericks, Ph.D., graduate program director in the school of nursing at Ryerson University in Toronto, who has conducted extensive research on cardiac care.

At her hospital, patients can see where they will have surgery and where they will recover, and connect with former patients, she says.

Doctors should also discuss the potential benefits of the surgery but be honest about the risks, too, so patients can make fully informed decisions.

The Society of Thoracic Surgeons , the main professional group that represents heart surgeons, provided Consumer Reports with the data used in our heart hospital ratings and has a risk calculator at riskcalc.sts.org. Use it with your doctors help, and ask him or her to carefully review the results with you.

From her research, Greenberg was prepared to face the post-op pain, which actually proved reassuring. When waking up from surgery, she remembers thinking with relief, Well, I cant be dead, because I am in terrible pain.

More important, she knew that her pain would soon diminish and that in time she would be stronger, with the prospect of many vigorous years ahead.

Yankees Lefty James Paxton Has Back Surgery Out 3

New York Yankees left-hander James Paxton has had back surgery and is expected to be sidelined until May or June

All during the offseason, New York Yankees general manager Brian Cashman worried about James Paxton’s back pain and kept suggesting: Let’s just get the surgery out of the way. We’re losing time.

Paxton was examined by three doctors, who said the issue could resolve itself.

They all came to the same conclusion: No, no, no, Cashman recalled.

But Paxton did not get better and finally had the operation Wednesday. He is expected to be sidelined until May or June.

New York said Dr. Andrew Dossett in Dallas performed a microscopic lumbar discectomy, in which a herniated disk is repaired, and removed a peridiscal cyst. The Yankees estimated Paxton will be able to start throwing in about six weeks and could pitch in a major league game in three-to-four months.

Paxton left his final regular-season start, at Texas on Sept. 27, after one inning with what the Yankees said was a tight left glute muscle. That ended Paxton’s career-best streak of victories in 10 consecutive starts.

Paxton was examined then by Yankees head team physician Dr. Christopher Ahmad and Rangers physician Dr. Keith Meister, and the pitcher received an injection of painkiller. Paxton went 1-0 with a 3.46 ERA in three postseason starts, allowing five runs in 13 innings.

Paxton was examined by Dossett, received a new round of injections and was told to see how he felt in three weeks.

Lesson #: There Is An Art And Science To Heart Valve Surgery

A diseased mitral valve can be surgically treated in a number of ways. Oftentimes the mitral valve can be repaired, meaning that the patients own tissue is used to reshape and modify the valve leaflets. The other alternative is an artificial valve replacement using either a mechanical prosthesis or a bioprosthesis from a cow or pig.

The fact that Mr. Paxton had to go back to surgery again for a tune-up implies that he originally had a valve repair and that the repair needed to be adjusted. A second open heart surgery rather soon after the first procedure does statistically increase the chances for a stroke to occur.

Bill Paxton Family Sues Hospital Doctor For Wrongful Death

Robotic heart surgery, heart valve disorders, and coronary arteries: Expert Q& A

LOS ANGELES The family of Bill Paxton has filed a wrongful death lawsuit against a Los Angeles hospital and the surgeon who performed the actor’s heart surgery shortly before he died.

The suit filed Friday against Cedars-Sinai Medical Center alleges the surgeon, used a “high risk and unconventional surgical approach” that was unnecessary and that he lacked the experience to perform, and that he downplayed the procedure’s risks.

And it alleges the hospital knew the surgeon, Dr. Ali Khoynezhad, tended to “engage in maverick surgeries and show suboptimal judgment.”

The misguided treatment caused Paxton to suffer excessive bleeding, cardiogenic shock and a compromised coronary artery, the suit alleges.

Actor Bill Paxton’s Stroke During Heart Surgery: What Were The Odds

UPDATE: Bill Paxton’s death certificate states the actor died last month from a stroke days after surgery to replace a heart valve and repair damage to his aorta, the Associated Press wrote on Monday.

Details are scarce, but in an otherwise healthy man of that age, the risk of stroke during heart-valve surgery is less than 1 percent, according to a risk calculator from the Society for Thoracic Surgeons.

Paxton, whose death was announced Sunday in a statement from family representatives, said earlier this year that he had suffered valve damage from a case of rheumatic fever as a child.

Rheumatic fever can damage the heart’s mitral valve, and less commonly the aortic valve, said Kimberly Campbell, a partner with Cardiology Consultants of Philadelphia.

This type of fever has become less common due to antibiotic therapy that is administered for strep throat, Campbell said. If strep isn’t properly treated, it can lead to rheumatic fever.

Here is what we know: Mr. Paxton, age 61, reportedly passed away from a massive stroke at some point in time following open heart surgery to treat a diseased heart valve. The surgery, Paxtons family claims, contributed to the post-surgical complications that led to stroke and the actors death.

The suit further claims that Khoynezhad was not in the hospital as Paxton suffered complications, causing a delay in treatment.

Bill Paxton and his family trusted the physicians and staff at this medical facility but instead Cedars-Sinai betrayed their trust, said Paxton family attorney Bruce Broillet in a statement to press. The surgeons actions resulted in this tragic and preventable death. The statement says Khoynezhad left his position at Cedars-Sinai shortly after Paxtons death.

In a press statement, the hospital said, State and federal privacy laws prevent us from commenting about patient care without written authorization. But we can share the following: Nothing is more important to Cedars-Sinai than the health and safety of our patients. These remain our top priorities. One of the reasons for our high quality is that we thoroughly review concerns about any patients medical care. This process ensures that we can continue to provide the highest quality care.

Actor Bill Paxton’s Death Prompts Thoughts About Surgery Risks

Day in the Life: Heart Surgeon

If you’re like a lot of people, you feel like you’ve lost a friend when an actor whose work you’ve enjoyed over the years dies, especially if the death was sudden. And if the actor is younger than you or not much older, you might find yourself thinking about your own mortality.

Bill Paxton, of course, is the latest popular actor to die unexpectedly. Count me among his many fans–I binge-watched his HBO series Big Love and have seen Titanic at least three times over the years . Paxton died over the weekend from complications of surgery, and if fans’ online comments about his death are any indication, people scheduled to undergo surgery themselves are worried.

A statement from his family did not go into detail about the type of surgery Paxton underwent, but in a podcast that aired Feb. 6, he told host Marc Maron that he had been diagnosed with rheumatic fever when he was in 7th grade .

Justin Cornwell, left, and Bill Paxton present the award for outstanding actress in a motion picture… at the 48th annual NAACP Image Awards on Feb. 11, 2017, in Pasadena, Calif. Cornwell co-starred with Paxton, who died only two weeks after this photo was taken, in the new CBS series “Training Day.”

Paxton explained that rheumatic fever results from untreated strep throat and said one of his first symptoms was wrist pain . “But it usually damages your heart valves,” he added.

“Did it?” Maron asked.

“Well, yeah, yeah,” Paxton replied.

Lesson #: There Are Many Different Types Of Strokes

A stroke is not a heart attack.

A stroke refers to blood flow to the neurons of the brain and a heart attack refers to a disruption of blood flow to the heart muscle.

There are two types of strokes. An ischemic stroke and a hemorrhagic stroke. An ischemic stroke refers to a lack of blood flow to the brain tissue and a hemorrhagic stroke refers to bleeding within the brain that damages brain tissue.

The ischemic stroke can be embolic, meaning that a piece of plaque or calcium or clot breaks off near the heart and gets caught-up in the blood stream. The piece of debris is then swept into the small vessels of the brain and literally plugs an artery – shutting off blood flow to the region of the brain supplied by that particular artery that is blocked.

Another type of ischemic stroke refers to a disruption of blood flow in the microvascularature of the brain at the microscopic level.

Bill Paxton: What Went Wrong

Surgeries of any kindand especially those on the heartcome with inherent risks and can be quite dangerous. Paxton, like anyone whos undergone surgery, was reportedly quite nervous leading up to the procedure, but theres no way he or his family could have predicted this outcome. Whats even worse is that Bill Paxtons death may have been preventable I remember a 2008 study reporting that approximately one-third of in-hospital coronary artery bypass graft deaths are avoidable.

And although I dont know when Paxton underwent surgery and what specifically it was for, I do know that death is a legitimate risk for those trying to recover.

Relatives of Aliens star say that medical facility was negligent in its care of actor

The wife and children of deceased actor Bill Paxton have filed a lawsuit against Cedars-Sinai Medical Center, alleging that the hospital was negligent in treating the Aliens actor prior to his death last year

According to the suit, filed by Paxtons wife Louise and children James and Lydia, the actor consulted the defendants over his cardiac conditions, including bicuspid aortic valve and aortic aneurysm, and was counseled to undergo heart surgery.

However, the suit contends, prior to Paxtons Feb. 14, 2017 surgery, the defendants misrepresented and/or concealed information relating to the risks of surgery and care that would be provided and/or failed to adequately explain the proposed treatment or procedure and/or failed to disclose that Defendant Ali Khoynnezhad, M.D., was going to use a high risk and unconventional surgical approach with which he lacked experience and which was, based on information and belief, beyond the scope of his privileges.
