Current NICE guidance on chest pain recommends that patients with a suspected heart attack should receive an ECG and high sensitivity troponin test on hospital arrival. If patients do not have the typical ECG features of a heart attack , a repeat high sensitivity troponin three hours later can confirm or rule out non-ST elevation heart attack.
Troponin measurement is not recommended for people presenting with chest pain who are not clinically suspected of having a heart attack.

In assessments of chest pain, cardiac troponins have a prominent position. In clinical work we are well aware that conditions other than myocardial infarction can result in an elevated troponin level, but the possibility of a falsely elevated test result appears to be little known.
In this article I present a case history that illustrates the problem of falsely elevated troponin levels and the consequences of disregarding this potential interpretation.
To diagnose a myocardial infarction, an increase and/or fall in troponin values by at least one value > the 99th percentile in a healthy reference population is required, combined with other ischaemic signs or symptoms . Troponin T and troponin I are considered equal in diagnostics of myocardial infarction. There is only one manufacturer of tests for troponin T and thus only one reference range. For troponin I, there are a number of manufacturers of tests with varying reference ranges.
The amount of change needed to make a diagnosis of myocardial infarction has been a matter of debate. It has been concluded that a 50% change for values around the 99th percentile is required, and 20% in case of higher levels of troponin .
Often Dismissed As Benign Troponinemia High Troponin Despite No Underlying Cardiac Renal Or Other Conditions Means A Worse Prognosis
A new Swedish analysis should serve as a reminder to physicians: elevated cardiac troponin, even in the absence of standard cardiac and noncardiac diseases known to cause a troponin rise, remains an important marker of increased risk. This is particularly important given the oft-heard concerns that the advent of high-sensitivity cardiac troponin assays have led to a glut of positive results that, in turn, have led to expensive, unnecessary tests and follow-up in people who dont need it.
On the contrary, write Kai Eggers, MD, PhD , and colleagues, more than 15% of patients with an initially elevated troponin who are discharged without a diagnosis related to their test results go on to die or have a major cardiovascular event in the next 5 years.
These patients deserve appropriate evaluation and if you do not find any clear-cut conditions explaining troponin leakage, then at least the value that you’re getting should be recognized as a signal that something wrong is going on in the heart, and that these patients at least should be targeted for appropriate follow-up, Eggers told TCTMD. Don’t discharge these patients without having made up a plan for them.

Causes Of Non Acs Related Troponin Elevations
Measurement of cardiac troponin has revolutionized the evaluation and management of patients with suspected acute coronary syndrome . Recent consensus statements endorse the use of cTnI or cTnT as the biomarker of choice for such application.1 In order to maximize sensitivity for diagnosis of ACS, as well as for superior prognostication, these same consensus statements endorse the use of the lowest cTn cut-off point, down to the 99th percentile of a normal population as long as the assay used delivers imprecision < 10% at this cut-off point.1 While the most widely-used conventional cTn methods cannot deliver this combination of sensitivity and precision, novel very high sensitivity cTn assays have now been developed; these assays have a greatly improved detection limit with improved precision.2 hsTn assays are soon to be more widely used.
Despite the value of cTn for ACS diagnosis, risk stratification and management, it has become quite evident that non-ACS elevation of the marker is not uncommon. In a recent study examining 69,299 patients admitted through the emergency department, 48% had their cTn measured. Of these, 2,344 patients had an elevated cTn concentration. Of those with a positive cTn, 42.7% of the patients did not have ACS.3
What Did This Study Do
This review identified 11 cohorts including 9,241 adults presenting to the emergency department with chest pain. All received an ECG and high sensitivity troponin T test.
Researchers looked at whether a troponin level of less than 0.005 µg/L when combined with normal ECG, was accurate in ruling out a heart attack. Heart attack was confirmed, independently in most studies, according to the Universal Global Task Force definition. Other outcomes were death or major adverse cardiac event within 30 days.
Nine of the 11 studies had a high or unclear risk of bias. Individual studies varied in patient characteristics, timing and number of troponin tests, and their results. These factors may make meta-analysis inappropriate. All studies were from developed countries, but only one was from the UK.

Troponin levels typically start to elevate in the circulation within 2 to 3 hours of the onset of chest pain. The levels will continue to rise at that time until a peak is reached, generally between 12 and 48 hours. The troponin level will then begin to fall over the next 4 to 10 days down to a normal level.

Patients Who Present In The Emergency Room With Chest Pain Are Given A Troponin Test
Troponin is a protein enzyme that leaks from the heart when this muscle is damaged.
If the test result is elevated , this can mean that the patient had a heart attack very recently.
However, elevated troponin doesnt always mean cardiac damage.
In fact, the test result isnt either normal or elevated; there is another range an in-between range.
Its sometimes referred to as a gray area or indeterminate area, which warrants a follow-up blood draw several hours later to see if the result is of a higher value .
When Is It Ordered
A troponin test will usually be ordered when a person with a suspected heart attack first comes into the emergency room, followed by a series of troponin tests performed over several hours.
A heart attack may be suspected and testing done when a person has signs and symptoms such as those listed below. Note that not everyone will experience chest pain, and women are more likely than men to have sign and symptoms that are not typical.
- Chest pain, discomfort and/or pressure
- Rapid heart rate, skipping a beat
- Shortness of breath and/or difficulty breathing
- Pain in other places: back, arm, jaw, neck, or stomach
In people with stable angina, a troponin test may be ordered when:
- Symptoms worsen
- Symptoms occur when a person is at rest
- Symptoms are no longer eased with treatment
These are all signs that the angina is becoming unstable, which increases the risk of a heart attack or other serious heart problem in the near future.

Blood Test And Ecg May Safely Rule Out Heart Attack
A high sensitivity troponin test accurately ruled out a heart attack amongst a third of patients presenting to the emergency department with chest pain. A patient with no detectable troponin and normal electrocardiogram was almost certain not to have had a heart attack.
Many people come to hospital with chest pain, but more than 75% of them have not had a heart attack. The two tests accurately ruled out heart attack in 30% of all chest pain presentations, but more than a third of people who didnt have a heart attack also tested positive. Only around a quarter of people with raised troponin have had a heart attack. The tests were less reliable in people who had chest pain for less than three hours.
The findings support existing NICE guidelines to use high sensitivity troponin testing in people with a suspected heart attack but without the classic features on an electrocardiogram. The test should not be used indiscriminately for all chest pain presentations.
Caution is needed due to the variability in individual study results, patient populations and testing protocols.
