Advanced Heart Failure And Cardiac Transplantation
The Advanced Heart Failure and Cardiac Transplant Team has the most active advanced heart failure program in the region, providing innovative medical and state-of-the art surgical solutions for patients with cardiomyopathy, heart failure, and cardiogenic shock. The program is the only program to offer durable mechanical circulatory left ventricular assist devices in the region, and has performed over 1,000 heart transplants in 30-plus years of clinical experience.
The Advanced Heart Failure program is actively engaged in performing clinical, basic and translational research, and is one of seven Regional Coordinating Centers for the National Heart Lung and Blood Institutes Heart Failure Clinical Research Network. The program also offers a one-year ABIM certified training program in advanced heart failure and cardiac transplantation.

Our training program prepares fellows for academic and clinical careers in the subspecialty of advanced heart failure and transplant cardiology. Fellows have access to a broad range of clinical experiences, advanced medical facilities, and research opportunities.
Our training program provides fellows with remarkable clinical exposure to mechanical circulatory support, orthotopic cardiac transplantation, endomyocardial biopsy, post-transplant surveillance, and care in multidisciplinary settings. Our program also offers support for scholarly activities, leadership training for management of heart failure programs, implementation of quality improvement and patient safety initiatives, and personal and professional development.

The Advanced Heart Failure and Transplant Cardiology Fellowship at NewYork-Presbyterian Hospital/Columbia University Irving Medical Center is a one-year education and training program in the management of all phases of heart failure and cardiac transplantation including the application and management of assist devices. Each July the program begins with a core curriculum lecture program targeted to the level of the advanced cardiology fellow. Over the course of 12 months, fellows learn about both inpatient and outpatient management. The program is accredited by ACGME.

Comprehensive Heart Failure Program
Led by Tariq Ahmad MD, MPH, the Comprehensive Heart Failure Program comprises of a number of out-patient and in-patient initiatives dedicated to producing high quality outcomes for all types of heart failure patients.
The Heart Failure Population Health Program is a clinical care network which extends across the Connecticut shoreline, from Greenwich in the west to New London in the east. Our group utilizes a variety of technologies for monitoring the health of our heart failure patients in the community, and we have several Heart Failure Disease Management clinics open to drive the medical care of patients with newly diagnosed heart failure or recent hospital admissions.
The Inherited Cardiomyopathy Program includes clinics staffed with clinicians whom are experts in diseases such as hypertrophic cardiomyopathy and arrhythmogenic cardiomyopathy. Cardiomyopathy clinics which focus on acquired disease such as cardiac sarcoidosis and cardiac amyloidosis also exist. Finally, directed by Robert Elder, MD, the Adult Congenital Heart Disease Program acts as a clinical care bridge for the pediatric patient with heart disease as they age and mature into adulthood.

One Year Clinical Training Program
The Advanced Heart Failure and Cardiac Transplant Fellowship is a 12-month training program with an emphasis on treating patients with advanced heart failure and those who have undergone cardiac transplantation or VAD placement.
This advanced training program consists of four full-time heart failure/transplant cardiologist along with a large multidisciplinary team of cardiac surgeons, pharmacists, nurse practitioners, LVAD and transplant nurses, social workers, physical and occupational therapist, and nutritionists.
Residents and general cardiology fellows also integrate with the team during their heart failure rotations. This multidisciplinary approach to each individual patient has allowed our excellent clinical outcomes.
The advanced heart failure/transplant fellows role includes leading the advanced heart failure team rounds, managing post-operative transplant and VAD patients, participating in the heart failure, VAD, and cardiac transplant clinics at Loyola, participating in the cardiac amyloid, infiltrative, hypertrophic cardiomyopathy and pulmonary hypertension specialty clinics at Loyola, performing right heart catheterizations, endomyocardial biopsies, participating at transplant committee meetings and quality improvement meetings and managing immunosuppression medications. Specific rotations can also be arranged based on any of the individual applicants specific interests.
Advanced Heart Failure And Transplant Cardiology Fellowship Program
The Advanced Heart Failure and Transplant Cardiology ACGME Fellowship program accepts one fellow per year who has completed an Internal Medicine residency and Cardiology fellowship. The Fellow will be training at the only site in South Jersey to perform durable Left Ventricular Assist Devices. Currently the Center implants approximately 15-20 per year. The Hospital has a robust cardiogenic shock program with excellent survival. In order to treat these complicated shock patients, Deborah has comprehensive treatment options such as Impella as well as the only regional center to have VA ECMO for long term support. Given our high volume cardiogenic shock program, Fellows will get extensive experience in bedside Swan-Ganz catheter placement and management of these devices. In 2019 over 250 bedside swans were placed in our critical care unit.

Educational Programs Conferences & seminars
The Loyola Advanced Heart Failure Fellowship program curriculum follows the ISHLT Heart Failure and Transplant Medicine Core Competency Curriculum and ACC Clinical Competence Statement on AHFTC.
The advanced heart failure fellow participates in monthly conferences including journal club, case conferences, and didactics on selected topics in AHFTC.
In addition, the fellow is to actively participate in weekly multidisciplinary transplant committee meetings and conferences, and is expected to assist in the education of residents and fellows rotating through the heart failure services and clinics.

Objectives Of The Fellowship
Our fellowship program offers the entire breadth of clinical exposure from ICU, post-LVAD/transplant floors, cardiac catheterization lab, dedicated HF IMU and outpatient clinic setting. Specialized aspects of our clinical training, in addition to MCS/transplant, include pulmonary hypertension, cardiac amyloidosis, multi-organ transplant , cardio-oncology and HFpEF. Our program is unique in a sense that fellows rotating through the cardiac catheterization lab receive hands-on training in implantation of the CardioMEMS device and intra-aortic balloon pump placement via a novel approach utilizing axillary artery access. This is in addition to the ample amount of exposure provided in performing endomyocardial biopsies, coronary angiography and invasive hemodynamic studies including exercise right heart catheterizations, constriction-restriction studies and LVAD speed optimization studies using concomitant RHC and TTE.
In summary, we prepare fellows for an academic career in advanced heart failure and transplant cardiology who we hope will eventually be future leaders in the field of heart failure.
Heart Failure Cardiac Transplant And Mechanical Circulatory Support
The Heart Failure, Cardiac Transplant and Mechanical Circulatory Support affinity group at Stanford consists of a group of physicians who specialize in a variety of subspecialties of heart failure management and work in an integrated manner with nurses, dietitians, pharmacists, and social workers to provide highly specialized and comprehensive care to patients with heart failure, and offer cardiac transplantation and mechanical circulatory support for those with end-stage disease.
Heart Failure Clinics
The Stanford Center for Inherited Cardiovascular Disease is led by Dr. Euan Ashley and encompasses a diverse group of physicians and genetic counselors who treat patients with genetic myopathies such as hypertrophic cardiomyopathy, arrhythmogenic right ventricular dysplasia, channelopathies and Marfan syndrome. The group has now become the largest hypertrophic cardiomyopathy center west of the Rockies.
The Stanford Amyloid Center is headed by Dr. Ronald Witteles who works in close collaboration with his colleagues in hematology to provide comprehensive evaluation and care for patients with amyloid heart disease. Dr. Witteles also specializes in the treatment of patients with cardiac sarcoidosis and chemotherapy-related cardiomyopathy.

Advanced Heart Failure And Transplant Cardiology
The University of Texas Southwestern Medical Center is pleased to offer two ACGME-accredited positions for a twelve-month fellowship in Advanced Heart Failure and Transplant Cardiology. Our training sites include the 700-bed William P. Clements Jr. University Hospital, and Parkland Memorial Hospital, Dallas Countys 850-bed public medical center.
Our academic heart failure and transplant program has provided advanced care for nearly three decades. UTSW offers subspecialty clinics in cardiac sarcoid, cardiac amyloid, neuromuscular cardiomyopathy, hypertrophic cardiomyopathy, adult congenital heart disease, pulmonary hypertension, heart failure with preserved ejection fraction, and cardio-oncology. Our program provides training in non-invasive and invasive cardiopulmonary exercise testing . We have a robust histocompatibility & immunogenetics laboratory Lab with mentorship by our Immunology colleagues. These rotations allow customization of training and exposure depending on a candidates individual interests.
Our mechanical circulatory support program implants the HeartMate 3 left ventricular assist device as both destination therapy and as a bridge-to-transplant. We continue to support patients previously implanted with the HeartMate II and HeartWare LVADs.

The Advanced Heart Failure and Transplant Cardiology Fellowship program, offered through MedStar Washington Hospital Center and Georgetown University, provides fellows with exceptional clinical training and research opportunities. Our program is based at MedStar Heart and Vascular Institute, which is located on the campus of MedStar Washington Hospital Center, and is partnered with Georgetown University School of Medicine.
The Advanced Heart Failure and Transplant Cardiology Fellowship is a one-year program and is ACGME accredited in good standing since its inception in 2013. We accept two fellows each year, balancing the optimal clinical exposure with a high faculty-to-fellow ratio.

Welcome to the Advanced Heart Failure and Transplant Fellowship Program at Temple University Hospital!
Temple is a pioneering heart transplant program with over 1000 transplants performed. Temple offers a collegial working relationship with a dedicated team of 8 HF/Transplant/VAD/PH cardiologists, 5 Transplant/VAD surgeons and 4 nurse practitioners. Our faculty members have broad research interest in outcomes, basic sciences, and clinical trials.
This is a one-year training program in advanced heart failure, pulmonary hypertension, cardiac transplantation and mechanical circulatory support. The curriculum will include patient management in the outpatient, inpatient and catheterization laboratory settings. Experience in transplant infectious disease, palliative care, imaging, device management, transplant immunology & pathology, cardiac rehab and cardiopulmonary exercise testing is included.

Program History And Overview
Under the direction of Michael Dickinson, MD, the advanced heart failure section was developed soon after the opening of the Frederick Meijer Heart and Vascular Institute in 2004. In 2008, Asghar Kaghani, MD, a world leading thoracic transplant surgeon, with more than 25 years of experience at Harefield Hospital , was named the surgical director of the Richard and Helen DeVos Heart and Lung Transplant program. Under this leadership, the left ventricular assist device program was initiated in 2009 and the first heart transplant occurred in 2010. Since then, we have implanted more than 450 LVADs and performed more than 185 heart transplants. The Frederick Meijer Heart and Vascular Institute is a quaternary referral center for West Michigan and beyond.

The Stanford Advanced Heart Failure Transplant Cardiology fellowship program is a one-year training program that follows an academic calendar . At the end of the program, graduating fellows will qualify to serve as medical directors of UNOS-certified heart transplant programs and will be eligible for certification in Advanced Heart Failure and Transplant Cardiology by the American Board of Internal Medicine .
Our program was among the first in the country to be certified by the Accreditation Council for Graduate Medical Education , with accreditation starting in July 2013. We accept three fellows per year, and candidates must have completed general cardiology training at an ACGME-accredited program.
Heart Transplantation & Ventricular Assist Devices
St. Vincent Heart Center of Indiana remains the largest and most active cardiac transplant program in the state of Indiana. In 2011, we performed 24 heart transplants and 64 ventricular assist device procedures, both increases over 2010.
In February 2012, St. Vincent celebrated the 25th anniversary of our first heart transplant, performed by Dr. John Paris. Today, as the largest heart transplant program in the state, we are helping more patients with cardiomyopathy, congenital heart defects, heart valve disease, debilitating arrhythmia and other heart conditions live more active, fulfilling lives.

Training And Procedure Requirements
The training pathway involves satisfactory completion of four years of training 36 months of accredited cardiovascular disease fellowship, including 24 months of full-time clinical training, plus 12 months of clinical advanced heart failure and transplant cardiology fellowship training* that meets the following criteria:
- Advanced heart failure and transplant cardiology fellowship training undertaken July 1, 2014 and thereafter must be accredited by the Accreditation Council for Graduate Medical Education . Advanced heart failure and transplant cardiology fellowship training taken prior to July 1, 2014 must be conducted within a program affiliated with an accredited cardiovascular disease fellowship program.
- For those institutions without a cardiac transplant center, program directors will need to establish a documented relationship with a transplant center at an affiliated institution in order to provide trainees with the required experience in the evaluation and management of pre- and post-operative cardiac transplant patients.
* Note: Advanced heart failure and transplant cardiology fellowship training cannot be used to fulfill the requirements for admission to the certification examination through the training pathway if that training has been credited toward the required 36 months of cardiovascular disease training. Total training must be four years.
Advanced Heart Failure & Transplant Cardiology Interview Process
St. Vincent Hospital and Heart Center of Indiana is recruiting eligible candidates for the 2021 2022 Advanced Heart Failure and Transplant Cardiology Fellowship Program. The fellowship program is an ACGME accredited 1-year program. The curriculum will include patient management in the outpatient, inpatient and catheterization laboratory settings. Experience in transplant infectious disease, imaging, and device management is included.
Candidates must have completed an ACGME approved 3-year Cardiology Fellowship to be considered.
Applications are currently being accepted for one position, via ERAS, for the 2021 2022 academic year.
