What You Should Do

Consult with your healthcare professional if you are considering stopping or reducing your dose, as abruptly stopping or reducing your dose may cause side effects such as dizziness, unusual dreams, electric shock sensations, agitation, anxiety, difficulty concentrating, migraine, headache, shakiness, sweating, nausea, or vomiting.

If you experience any symptoms of abnormal heart rhythms such as heart palpitations, dizziness, fainting, or seizures while taking Cipralex, contact your healthcare professional immediately.

Patients with questions or concerns about their Cipralex treatment should speak to a healthcare professional.

Managing Common Side Effects

Generally, these common side effects are mild and can be managed. Your doctor may recommend strategies like:

  • Changing the timing of dosesfor example, taking your medication after meals if you experience gastrointestinal symptoms, before bed if you have insomnia, or after sex if you’re experiencing sexual side effects
  • Increasing your physical activity to help manage anxiety, sleep problems, or constipation

The TGA is advising that citalopram should no longer be used at doses greater than 40 mg per day. For some patients, the maximum recommended dose is now only 20 mg per day .

In addition, it is now advised that people who have been diagnosed with a heart condition known as “congenital long QT syndrome” should not take citalopram.

A recent study by the Forest Research Institute in the USA has shown that higher doses of citalopram can cause abnormal changes in the electrical activity of the heart. In rare cases these changes can result in serious heart problems.

Citalopram is a member of the class of drugs called selective serotonin reuptake inhibitors and is used to treat major depression. Brand names include:

  • Cipramil
  • others with the word “citalopram” in their name

When To Notify Your Doctor

While these side effects usually do not require medical intervention, you should report them to your healthcare provider if they continue for over one week or interfere with your daily routine. Your doctor;may decide to adjust your dosage or switch you to another medication entirely.

Also, you may experience other less common side effects. If you do, alert your doctor. You should never;stop or change the dosage of your medication on your own.

Managing Patients Currently On A Less Preferred Antidepressant

Managing patients with a new diagnosis of CHD currently on an antidepressant requires care. You should:

  • Seek advice from a mental health specialist before switching.
  • Take into account the extent and severity of CHD and other medical history
  • Consider the persons preferences and past experience with antidepressant use if any
  • Check cautions, contraindications relating to the cardiovascular system and any drug interactions .

Switching from less to more preferred options

Switching patients with CHD from their current antidepressant to another antidepressant may sometimes be necessary. Our advice on antidepressant switching may be helpful.

Other Serious Side Effects

Lexapro can also cause other serious side effects. Call your doctor right away if you have serious side effects. Call 911 or local emergency services if your symptoms feel life-threatening or if you think youre having a medical emergency.

Severe allergic reactions

You should not take Lexapro if youre allergic to it, its ingredients, or the antidepressant Celexa. Symptoms can include:

  • breathing trouble
  • swelling of your face, tongue, eyes, or mouth
  • severe rash, hives , or blisters that may come on with fever or joint pain

Seizures or convulsions

There have been reports of some people having seizures while taking Lexapro. People with a history of seizures are at higher risk.

Serotonin syndrome

This is a serious condition. It happens when the levels of serotonin in your body get too high. Its more likely to occur if you also take other drugs that increase serotonin, such as other antidepressants or lithium. Symptoms can include:

  • agitation
  • talking quickly or more than normal

Vision problems

Lexapro may dilate your pupils. This may trigger a glaucoma attack, even if you dont have a history of eye problems. Symptoms can include:

  • eye pain
  • swelling or redness in or around your eye

The recent study found dose-dependent QT prolongation with citalopram. As higher doses may result in life-threatening or fatal arrhythmias in some people, the following new dosage recommendations and contraindications are advised:

The maximum recommended daily dose of citalopram is now 40 mg.

A maximum recommended daily dose of 20 mg now applies to patients:

  • greater than 65 years of age
  • with liver dysfunction
  • with poor CYP2C19 activity or
  • taking concomitant CYP2C19 inhibitors .

In patients aged over 65 years, the recommended starting dose is now 10 mg daily.

Citalopram should not be used in patients with congenital long QT syndrome.

Citalopram should be used with caution in patients at higher risk of developing prolongation of the QT interval, including those with:

  • congestive heart failure
  • concomitant medicines that prolong the QT interval.

There are new monitoring recommendations for patients on citalopram:

  • Hypokalaemia and hypomagnesaemia should be corrected prior to initiation of treatment and potassium and magnesium levels should be periodically monitored.
  • More frequent ECG monitoring should be considered for patients at higher risk of QT prolongation.

If citalopram is discontinued or the dose reduced, the patient should be monitored closely for the re-emergence or worsening of any symptoms of depression.

Major depressive disorder is one of the most debilitating health conditions in the world that often requires lifelong treatment and close monitoring. The World Health Organization now reports MDD to be the universal leading cause of disability.1 Untreated MDD has been associated with an increased risk of suicide, decreased quality of life and increased risk of cardiovascular related mortality.2 The increased cardiovascular disease risk can also be attributed to other factors, including physical inactivity, poor medication adherence, smoking, obesity and other major comorbidities such as diabetes and hypertension .3 There is also a high prevalence of MDD in patients with pre-existing CVD. Up to 45% of patients with coronary artery disease , including those with stable CAD, unstable angina or myocardial infarction suffer from clinically significant depressive symptomatology.4 The following review will discuss the treatment of MDD related to CVD and evaluate antidepressant use in relation to the updated guideline recommendations regarding the treatment of heart failure and HTN.

    Antidepressant Escitalopram Helps Heart Problem Caused By Stress

    Researchers at Duke Medicine have revealed that a commonly prescribed antidepressant escitalopram could be used to treat a heart condition caused by stress among people suffering from stable coronary heart disease.

    The findings of the study, published in the May 22/29, 2013 issue of the Journal of the American Medical Association, found that people with stable coronary heart disease and mental stress-induced myocardial ischemia taking escitalopram a selective serotonin reuptake inhibitor used to treat depression and anxiety experienced lower rates of MSIMI.

    In addition, the medication has been shown to significantly improve the quality of life among those suffering from anxiety disorders.

    In MSIMI there is limited blood flow to the heart; it has been shown in previous studies that emotional stress can trigger heart conditions. This is of particular concern because patients with MSIMI tend to experience worse heart problems.

    Lead author, Wei Jiang, M.D., associate professor of psychiatry and behavioral sciences and internal medicine at Duke, said:

    Mental stress-induced myocardial ischemia is a serious condition, as patients with the condition tend to have worse heart problems compared to patients without it. This study showed for the first time that it is treatable with an emotion-modulating medication.

    There arent many options currently available to treat the condition.

    There is strong evidence that patients with major depressive disorder are at increased risk of developing coronary heart disease . This elevated risk is independent of classical risk factors such as smoking, obesity, hypercholesterolemia, diabetes and hypertension. The risk of CHD is increased 1½-2 fold in those with minor depression and 3-4½ fold in subjects with MDD. Put simply, the relative risk of developing CHD is proportional to the severity of the depression. While the mechanism of increased cardiac risk attributable to MDD is not known disturbances in autonomic function most likely do play a part.
