That means your breathing could become so slow and shallow that you end up with too much carbon dioxide in your blood and not enough oxygen. Its a problem that has the potential to be fatal.
The FDA is now requiring manufacturers of these drugs to add a statement about the risk of respiratory depression to the prescribing information that patients receive with their prescription. They are also requiring the manufacturers to conduct research to determine whether these anticonvulsants are frequently abused/misused and whether inappropriate use increases the likelihood of respiratory side effects.
While this news is certainly worth taking seriously, you should know that relatively few adverse events have actually been reported to the FDA, and those that occurred impacted people who already had known respiratory issues.

Our evaluation shows that the use of these medicines, often referred to as gabapentinoids, has been growing for prescribed medical use, as well as misuse and abuse. CNS depressants include opioids, anti-anxiety medicines, antidepressants, and antihistamines. We initially investigated the central cardiovascular effects of gabapentin in the SHR rats by microinjection into the NTS. Unilateral microinjection of increasing doses of gabapentin into the NTS produced dose-dependent depressor and bradycardic effects . Microinjection of gabapentin at a dose of 3.03 nmol did not change the BP or HR in the SHR rats. A dose of 10 nmol slightly reduced the BP and HR , while microinjection at a dose of 33 nmol induced dose-dependent increases in the SHR rats . Therefore, 33 nmol of gabapentin was chosen in subsequent studies to investigate the central cardiovascular effects of gabapentin in the NTS of the SHR rats.

What Should Health Care Professionals Do
Health care professionals should start gabapentinoids at the lowest dose and monitor patients for symptoms of respiratory depression and sedation when co-prescribing gabapentinoids with an opioid or other central nervous system depressant such as a benzodiazepine. Patients with underlying respiratory disease and elderly patients are also at increased risk and should be managed similarly.
We recognize that incorporating one or more medications with non-drug therapies is the prevailing approach for optimizing analgesia. However, pairing an opioid with any CNS depressant â a gabapentinoid, benzodiazepine, sedating antidepressant, sedating antipsychotic, antihistamine, or other product â will increase the risk of respiratory depression. Shifting treatment from one CNS depressant to another may pose similar risks. Be aware of the potential additive effects of all these CNS depressants and plan accordingly, by starting with low doses, titrating carefully, and informing patients of the potential for CNS and respiratory depression and their symptoms. The gabapentinoid prescribing information already includes guidance for health care professionals to caution patients about dizziness, somnolence, and the potential for impaired ability to operate a car or complex machinery.

As demonstrated in a report from the;Medical Psychiatry Series, recent studies and practices have indicated that gabapentin can also be useful in treating a variety of psychiatric disorders, such as anxiety and depression. In addition, several trials have been performed that show gabapentin can be useful in helping people in addiction treatment to detox, particularly in withdrawal from alcohol and other depressants, like benzodiazepines.
A study from;JAMA Internal Medicine;shows that people recovering from alcoholism who used gabapentin were more likely to either fully abstain from or at least avoid heavy use of alcohol. |Drug: Gabapentin Enacarbil Drug: Placebo||Phase 4|
The study design is a phase IV single center, single blind, placebo run-in fixed dose single group study in twenty subjects with moderate to severe restless leg syndrome. It includes a one- three week screening washout period, one week of placebo run-in and four- week treatment period with GEn 600 mg once daily. Subjects will be instructed to take their study medication once daily with food in the evening at approximately 5 PM. If the dose is not taken at the recommended time, the next dose should be taken the following day at the regularly scheduled time . Subjects will be blinded to treatment .
The visit schedule includes: Screening; Scheduled Tests/Exams; Placebo administration visit; Baseline PSG visits; Baseline Visit; End of treatment PSG visits; Adverse Event Evaluation ;Unscheduled clinical evaluation visit; and End of Study Visit.
