Teaching Hospital In Calabar Performs First Open
The University of Calabar Teaching Hospital on Wednesday successfully performed its first open-heart surgery which lasted for six and half hours.
The Chief Medical Director/Chief Executive Officer of the hospital, Prof Ikpeme Ikpeme, in a statement annoucing the feat gave details of the preparations that were made to ensure the success of the operation.
The statement read in part, Today, Wednesday 15th September 2021, at 6.43 pm, history was made in our UCTH when the first-ever open heart surgery was performed in our hospital and the patient safely wheeled out to our Intensive Care Unit.;
She had received a Mitral valve replacement following about six and a half hours of surgery. This has been the culmination of two years of hard work, determination, sacrifice, vision and focus at different levels of our hospital. And we must offer sincere gratitude to God and all who worked to make this happen.;
It is the guiding principle of this management to work quietly to achieve the vision and mission of our hospital and reposition her appropriately in the comity of tertiary health institutions in Nigeria.
We have driven this agenda by focusing steadily on the 3R mantra and refusing to be distracted. We must appreciate all staff who made this happen openly.

Heart Valve Repair Or Replacement
For the heart to work well, blood must flow in only one direction. The heart’s valves make this possible. Healthy valves open and close in a precise way as the heart pumps blood.
Each valve has a set of flaps called leaflets. The leaflets open to allow blood to pass from one heart chamber into another or into the arteries. Then the leaflets close tightly to stop blood from flowing backward.
Heart surgery is used to fix leaflets that don’t open as wide as they should. This can happen if they become thick or stiff or fuse together. As a result, not enough blood flows through the valve.
Heart surgery also is used to fix leaflets that don’t close tightly. This problem can cause blood to leak back into the heart chambers, rather than only moving forward into the arteries as it should.
To fix these problems, surgeons either repair the valve or replace it with a man-made or biological valve. Biological valves are made from pig, cow, or human heart tissue and may have man-made parts as well.
To repair a mitral or pulmonary valve that’s too narrow, a cardiologist will insert a catheter through a large blood vessel and guide it to the heart.
The cardiologist will place the end of the catheter inside the narrow valve. He or she will inflate and deflate a small balloon at the tip of the catheter. This widens the valve, allowing more blood to flow through it. This approach is less invasive than open-heart surgery.
Are There Alternatives To Standard Open
Thanks to medical advancements, many procedures that once required opening the chest can now take place using minimally invasive heart surgery or with small incisions. The surgeon sometimes still needs to cut through part of the breastbone .
Depending on your situation, your surgeon may be able to use these methods:
- Catheter-based: Your surgeon threads a catheter to the heart. The surgeon then inserts surgical instruments, balloons, or stents through the catheter to perform a procedure. Catheter-based procedures include transcatheter aortic valve replacement and coronary angioplasty and stenting.
- Video-assisted thoracic surgery : Your surgeon performs VATS by inserting a tiny video camera and surgical instruments into several small chest incisions. Your surgeon may use VATS to place a pacemaker, repair heart valves or treat an arrhythmia.

The terms single bypass, double bypass, triple bypass, quadruple bypass and quintuple bypass refer to the number of coronary arteries bypassed in the procedure. In other words, a double bypass means two coronary arteries are bypassed coronary artery and right coronary artery ); a triple bypass means three vessels are bypassed ); a quadruple bypass means four vessels are bypassed while quintuple means five. Left main coronary artery obstruction requires two bypasses, one to the LAD and one to the LCX.
A coronary artery may be unsuitable for bypass grafting if it is small , heavily calcified, or located within the heart muscle rather than on the surface. A single obstruction of the left main coronary artery is associated with a higher risk for a cardiac death and usually receives a double bypass.
The surgeon reviews the coronary angiogram prior to surgery and identifies the number of obstructions, the percent obstruction of each, and the suitability of the arteries beyond the obstruction as targets. The presumed number of bypass grafts needed as well as the location for graft attachment is determined in a preliminary fashion prior to surgery, but the final decision as to number and location is made during surgery by direct examination of the heart.

What Happens During Surgery
Open heart surgery is a complicated and time taking procedure lasting between 3- 6 hours, depending on the procedure that you are undergoing.;
As you are under general anesthesia, you will not feel anything. You will be placed under ventilator support to ensure normal breathing during the operation. Your surgeon will make an incision of about 6-8 inches in the center of your chest to access your heart. A vital tool used during open-heart surgery is the heart-lung bypass machine, which is connected to your heart. As your heart stops breathing during operation, this machine will take up the pumping action of your heart.;
Once the operation is over, the surgeon will remove the heart-lung bypass machine, and your heart starts beating normally and perform stitches on the incision to close the cut.;
Choice Of Source Of Grafts
The choice of vessel is highly dependent upon the particular surgeon and institution. Typically, the left internal thoracic artery is grafted to the left anterior descending artery and a combination of other arteries and veins is used for other coronary arteries. The great saphenous vein from the leg is used approximately in 80% of all grafts for CABG. The right internal thoracic artery and the radial artery from the forearm are frequently used as well; in the U.S., these vessels are usually harvested either endoscopically, using a technique known as endoscopic vessel harvesting , or with the open-bridging technique, employing two or three small incisions. The right gastroepiploic artery from the stomach is infrequently used given the difficult mobilization from the abdomen.

What Is Open Heart Surgery

How To Prepare For Open

In the two weeks before the surgery, your doctor may ask you to quit smoking and stop taking blood-thinning medications, such as aspirin, ibuprofen, or naproxen.
Its important to talk to your doctor about your alcohol consumption before you prepare for the surgery. If you typically have three or more drinks a day and stop right before you go into surgery, you may go into alcohol withdrawal. This may cause life-threatening complications after open-heart surgery, including seizures or tremors. What Is Hybrid Cardiac Surgery
Hybrid cardiac surgery combines cardiac surgery with an interventional approach. Hybrid procedures facilitate minimally invasive approaches to surgery in lieu of a traditional chest incision . In high-risk patients and patients undergoing repeat operations, risky surgical procedures may be converted to a less complex surgery combined with an interventional procedure. The following;is the general sequence of events that will most likely occur during robotic cardiac surgery:
- You may receive a sedative before the procedure to help you relax.

- A surgeon will make a series of keyhole-sized incisions on the side of your chest. These incisions will align with the openings between your ribs.
- You may need to be placed on the heart-lung machine for the procedure.
- Depending on the procedure being done, your surgeon will insert several precision-guided robotic arms into these incisions.
- These robotic arms hold and manipulate tiny instruments to do;the required tasks on the heart or surrounding arteries.
- A tiny video camera will be inserted into another incision to provide a magnified, three-dimensional image of the operating site.
- The surgeon will control the robotic arms and camera from a console located within the operating room.
- Once complete, the surgeon will remove the instruments and close the incisions.
