You should not use losartan if you are allergic to it.

If you have diabetes, do not use losartan together with any medication that contains aliskiren .

You may also need to avoid taking losartan with aliskiren if you have kidney disease.

Do not use if you are pregnant. If you become pregnant, stop taking this medicine and tell your doctor right away. Losartan can cause injury or death to the unborn baby if you take the medicine during your second or third trimester.

To make sure losartan is safe for you, tell your doctor if you have:

  • kidney disease
  • an electrolyte imbalance
  • if you are on a low-salt diet or
  • if you are dehydrated.

It is not known whether losartan passes into breast milk or if it could harm a nursing baby. You should not breast-feed while using this medicine.

AT1 receptors of Ang II are believed to mediate most of the physiologically important actions of Ang II, as the amount of AT2 receptors declines sharply after birth. In humans, losartan and irbesartan have demonstrated this effect, while at higher doses, valsartan and telmisartan have inhibited platelet aggregation. Candesartan does not appear to influence platelet aggregation. In the usual therapeutic dosing range, losartan has been shown to be effective. This effect is not seen with ACE inhibitors.

For people with diabetes and taking aliskiren: You should not take losartan if you have diabetes and are take a drug called aliskiren to reduce blood pressure. Taking both of these drugs can increase your risk of side effects from losartan. Side effects include low blood pressure, high potassium levels in your blood, and kidney damage. If you have diabetes and are taking aliskiren, talk with your doctor before starting losartan.

Blockade of the RAS system has been shown to improve survival and hospitalizations in heart failure patients being treated with ACE inhibitors. These benefits of treating with losartan have also been evaluated. The first trial, Evaluation of Losartan in the Elderly compared treatment of NYHA class IIIV heart failure patients treated with captopril and losartan treated for 47 weeks. The primary end point was a worsening renal function. There were no significant changes in renal function. It is important to note that the event rate was lower than anticipated, and therefore, the study may not have been powered to show a significant difference. As a secondary end point, mortality was decreased 46% in the losartan-treated patients. To show superiority of losartan, a second trial ELITE II enrolled 3,152 patients with NYHA class IIIV heart failure and a left ventricular ejection fraction of 40% or less. Patients were treated with either losartan 50 mg a day or captopril 50 mg 3 times a day. There were no statistical differences between the 2 treatment arms regarding the primary end points, including sudden death and the composite of mortality and hospitalizations. Losartan was better tolerated than captopril with fewer patients discontinuing prematurely owing to adverse events .

Doses were increased and hydrochlorothiazide or other medications were added to obtain a target blood pressure of less than 140/90 mmHg. Both medications were started at 50 mg and titrated to 100 mg as needed. The primary end points were occurrence of cardiovascular death, myocardial infarction, or stroke, and the composite end point was any of these events. Losartan was associated with a significantly decreased incidence of the primary composite end point. This was primarily due to a decrease in fatal and nonfatal stroke. Blood pressure control was similar in the 2 groups. This stroke benefit was not seen in African Americans.

Even with successful reduction of blood pressure, hypertensive patients have significantly increased rates of cardiovascular disease. Left ventricular hypertrophy is believed to be the crucial factor in this continued risk of morbidity and mortality and, in theory, blockage of angiotensin II could reverse LVH. Angiotensin II blocking drugs such as losartan could therefore have direct cardioprotective effects in addition to the effect of lowering blood pressure. Dahlöf and colleagues compared rates of cardiovascular morbidity and death in patients treated with losartan and patients treated with conventional antihypertensive therapy consisting of atenolol and diuretics.

They enrolled more than 9,000 Scandinavian patients 55 to 80 years of age who had hypertension and evidence of LVH on electrocardiography . Exclusions included patients with secondary hypertension, recent myocardial infarction or stroke, angina, heart failure, or contraindications to the study medications. Untreated blood pressures at the beginning of the study were in the range of 160 to 200 mm Hg systolic and 95 to 115 mm Hg diastolic. Patients were randomly assigned to treatment based on losartan or atenolol, and the study was double-masked. Treatment was adjusted to attain blood pressures of less than 140/90 mm Hg, and patients were followed clinically, including ECG monitoring, for at least four years.
