What Are The Types Of Open
There are two ways to perform open-heart surgery:
- On-pump: A heart-lung bypass machine connects to the heart and temporarily takes over for the heart and lungs. It circulates blood through the body while moving blood away from the heart. The surgeon then operates on a heart that isnt beating and doesnt have blood flow. After surgery, the surgeon disconnects the device and the heart starts to work again.
- Off-pump:Off-pump bypass surgery takes place on a heart that continues to beat on its own. This approach only works for coronary artery bypass grafting surgery . Your surgeon may call this beating-heart surgery.

Heart Bypasses Cost Billions But Theres Opportunity For Improvement
Coronary artery bypass expenses vary widely across hospitals, a new study finds. Addressing a few key areas could lower hospital bills and improve quality of care.
Add up the hospital bills for all the heart bypass operations performed in the United States every year, and the tab comes to about $6.7 billion. That doesnt include the cost of care that the more than 160,000 patients need after they leave the hospital.

As one approach to payment reform, the Centers for Medicare& Medicaid Services planned to start paying select hospitals and doctors in a new way in January. Called bundled payments, this was intended to encourage improved and efficient care across the 90-day post-surgery period after a coronary artery bypass graft, or CABG, operation.
But last week, the agency proposed to cancel the program and others like it. A public comment period runs until mid-October.
This week, by coincidence, a study of the payments for 5,910 CABG patients treated at 33 Michigan hospitals was published in JAMA Surgery by a University of Michigan team.
The main finding: The risk-adjusted, price-standardized payments for CABG care varied widely at both the patient and hospital levels across the 90-day period, which includes the operation, recovery and care for associated complications.
How Dangerous Is A Bypass Operation
Around 70,000 bypass operations are performed in Germany per year . Hence, the operation is a routine intervention. In some clinics, it is performed more than 1,000 times per year. However, it is still a major intervention. Like for every surgery, wound infection, secondary bleeding or the formation of blood clots can occur. Also, cardiac arrhythmia or heart failure can be the result of such an operation. Despite these possible complications, most patients overcome this intervention really well.

Coverage For Cardiac Catheterization
Medicare also covers minimally invasive heart disease treatments that may be a viable alternative to open heart surgery. These laparoscopic surgeries use small incisions and with the surgeon using tubes and small instruments to repair the blockages.
These procedures are collectively called cardiac catheterization because they involve catheters inserted into the affected heart arteries.
Cardiac Catheterization Alternatives to Open Heart Surgery
- Angioplasty
- Balloon angioplasty uses a catheter to place a medical balloon into the blocked artery. The balloon is inflated, it opens the narrowed artery and improves blood flow. This procedure does not always result in long-term improvement.
- Stents
- Similar to an angioplasty, a metal stent is inserted into the blocked artery. It is expanded, opening the blockage and holding the artery open for years or decades. This procedure can cause blood clots, which are often treated with prescription blood thinners.
Typically, cardiac catheterization is covered by Medicare Part B medical insurance. You are responsible for your Part B deductible. After that, Medicare pays 80 percent, and you pay 20 percent of the costs.
Medicare Part A hospital insurance covers the procedure if youre admitted to a hospital and remain an inpatient through at least two midnights in a row during which the procedure is performed.
How To Prepare For Open
Tell your doctor about any drugs you are taking, even over-the-counter medications, vitamins, and herbs. Inform them of any illnesses you have, including herpes outbreak, cold, flu, or fever.
In the two weeks before the surgery, your doctor may ask you to quit smoking and stop taking blood-thinning medications, such as aspirin, ibuprofen, or naproxen.
Its important to talk to your doctor about your alcohol consumption before you prepare for the surgery. If you typically have three or more drinks a day and stop right before you go into surgery, you may go into alcohol withdrawal. This may cause life-threatening complications after open-heart surgery, including seizures or tremors. Your doctor can help you with alcohol withdrawal to reduce the likelihood of these complications.
The day before the surgery, you may be asked to wash yourself with a special soap. This soap is used to kill bacteria on your skin and will lessen the chance of an infection after surgery. You may also be asked not to eat or drink anything after midnight.
Your healthcare provider will give you more detailed instructions when you arrive at the hospital for surgery.

Finally Some Data About Heart Valve Repair & Heart Valve Replacment Costs
It was quite frustrating and time consuming.
Eventually though, I found three references to heart valve surgery costs from Columbia University, The National Library of Medicine and East Carolina University. Here is what the references said:
- Columbia University. Between 1992 and 1997, length of hospital stay decreased from 13.4 to 8.0 days and cost decreased from $37,047 to $21,856. Similarly, between 1992 and 1997 for mitral valve repair, length of stay decreased from 15.6 to 8.1 days and cost decreased from $45,072 to $21,747. The net result over the time period from 1988 to 1997 was an average decline in the cost of operation of $785 a year, adjusted for other factors.
- National Library of Medicine. Cost per patient was $14,469 for mitral valve replacement, and $11,606 for mitral valve repair.
- At East Carolina University, surgeons say that the hospital gets paid a fixed fee of $25,000 for the total cost of each heart-valve repair regardless of whether robotics are used.
As you can tell, the cost estimates from the medical resources significantly varied from my friends estimates. My friends were telling me the procedure would cost over $100,000 while the medical journals were telling me costs for mitral valve repair would be around $25,000.
What Happens During Open
Heart surgery is complex. Some surgeries may take six hours or longer. You will receive anesthesia and be asleep during the procedure.
Surgery steps vary depending on the heart condition and procedure. In general, your surgeon:
- Makes a 6- to 8-inch long incision down the middle of your chest.
- Cuts the breastbone and spreads your ribcage apart to reach your heart.
- Connects the heart to a heart-lung bypass machine, if youll have an on-pump surgery. An anesthesiologist gives IV medication to stop your heart from beating and monitors you during the surgery.
- Repairs your heart.
- Restores blood flow to your heart. Usually, your heart starts beating on its own. Sometimes, the heart needs a mild electrical shock to restart it.
- Disconnects the heart-lung bypass machine.
- Closes the breastbone or other incision with wires or sutures that remain in your body.
- Uses stitches to close the skin incision.

Minimally Invasive Heart Bypass Surgery Costs
