Blood Test To Diagnose Heart Attacks Is Flawed Warn Researchers
One in 20 patients had test levels higher than recommended limit – study results could help to avoid misdiagnosis and inappropriate treatment
The blood test used to diagnose a heart attack in patients admitted to hospital can be misleading, warn researchers in a study published by The BMJ today.
Of 20,000 consecutive patients undergoing blood tests at University Hospital Southampton, one in 20 had levels of troponin greater than the manufacturers recommended upper limit. But in most of these patients were being seen for other conditions and so there were no clinical signs or symptoms of a heart attack.
Troponin levels also differed according to several factors such as age, sex and whether inpatient or outpatient.
The researchers, led by cardiologist Professor Nick Curzen, say these results highlight the need for medical staff to interpret troponin levels carefully in order to avoid misdiagnosis of a heart attack and inappropriate treatment.
Current guidelines recommend troponin tests to help exclude or diagnose a heart attack. Manufacturers of troponin tests provide a recommended level based on values from a few hundred healthy individuals.
This recommended level is used as the upper limit of normal . In other words, if the value of troponin is above the 99th percentile, that is considered to be abnormal, and would indicate a heart attack in appropriate clinical circumstances.
The average age of participants was 61 and 53% were women.
What Are The Implications
This meta-analysis generally supports NICE recommendations around the use of the high sensitivity troponin test in people with a suspected heart attack but without the classic ECG signs of ischaemia.
Although it is not reliable less than three hours after the onset of pain, where the ECG is normal the new high sensitivity assay may reduce waiting times and avoid unnecessary hospital admissions and associated costs.

Hereâs how to use an AED:
1. Check responsiveness
- For an adult or older child, shout and shake the person to confirm whether theyâre unconscious. Do not use AED on a conscious person.
- For an infant or young child, pinch their skin. Never shake a young child.
- Check breathing and pulse. If absent or uneven, prepare to use the AED as soon as possible.
2. Prepare to use AED
- Make sure the person is in a dry area and away from puddles or water.
- Check for body piercings or outline of an implanted medical device, such as a pacemaker or implantable defibrillator.
- AED pads must be placed at least 1 inch away from piercings or implanted devices.
3. Use AED
For newborns, infants, and children up to age 8, use a pediatric AED, if possible. If not, use an adult AED.

Mike Marber, professor of cardiology at King’s College London and head of the UK arm of the research, said: Weve shown that this test is not only just as good as the current test for working out who has had a heart attack, but its also much better at working out who hasnt.
Dr Tom Kaier, one of the lead researchers, said the new test could quickly reassure thousands of patients that they are fine and would help free up valuable hospital beds in A& E departments and wards across the country.

A Simple And Highly Sensitive Blood Test Can Detect If You Just Suffered A Heart Attackand This Test Is So Super Sensitive That It Can Detect Even A Mild Heart Attack
The blood test is for an enzymatic protein called troponin. The troponin blood test for heart attack is highly sensitive and is considered the gold standard for determining damage to cardiac muscle.
Cardiac enzymes are chemicals contained within heart cells, says Christopher J. Hanifin, PA-C, who was previously a physician assistant in open heart surgery with Cardiothoracic Surgery of South Bend in South Bend, IN.
Normally, they are essentially undetectable in the bloodstream, continues Hanifin.
When a person suffers a heart attack, cells in the heart die and rupture, releasing these enzymes into the blood where they can be detected.
Diagnosis of a heart attack is usually based upon the detection of elevated levels of these chemicals in the blood.
If you go into an emergency room complaining of current or recent chest pain, the doctor or physician assistant will order a blood draw to check your troponin levels.
So if a patient goes to the emergency department with chest pain and their cardiac enzymes tests are negative, are they in the clear? Not necessarily, says Hanifin.
It takes some time for cardiac enzymes to appear in the blood. If a patient is tested in the early stages of a heart attack, it is very possible that their levels will be within normal ranges.
However, an elevated troponin level alone should not be used to outright diagnose heart attack.
Its never elevated by accident. As mentioned, this blood test is super sensitive.

What Does Current Guidance Say On This Issue
Current NICE guidance on chest pain recommends that patients with a suspected heart attack should receive an ECG and high sensitivity troponin test on hospital arrival. If patients do not have the typical ECG features of a heart attack , a repeat high sensitivity troponin three hours later can confirm or rule out non-ST elevation heart attack.
Troponin measurement is not recommended for people presenting with chest pain who are not clinically suspected of having a heart attack.
Stress Ekg Or Echocardiogram
Stress tests are performed to see how the heart performs under physical stress. The heart can be stressed with exercise on a treadmill or in a few instances, a bicycle. If a person cannot exercise on a treadmill or bicycle, medications can be used to cause the heart rate to increase, simulating normal reactions of the heart to exercise.
During the stress test, you will wear EKG leads and wires while exercising so that the electrical signals of your heart can be recorded at the same time. Your blood pressure is monitored throughout the test. The stress test can be performed together with the echocardiogram, described above.
