Dying during or immediately after physical activity occurs rarely. But the exercise-related death of a prominent Wall Street executive last week nevertheless raises concerns for people who want to keep active during middle age and later years.
Regular exercise is a cornerstone of good health, and its long-term benefits for both longevity and protection against heart attacks, cancer and other ailments are supported in many studies. Doctors say there are strategies to reduce the already low likelihood of a workout turning into a tragedy.
More Frequent The Dose Lower The Risk Study Finds
Researchers looked at 16,500 men treated with PDE5 inhibitors with just under 2,000 of them receiving alprostadil. The data was collected from the patient, drug, and cause of death registries in Sweden.
Researchers showed that the men receiving PDE5 inhibitors not only lived longer, but also had a lower risk of new heart attack, heart failure, balloon dilation, and bypass surgery than patients receiving alprostadil.
According to the study, this protection was dose-dependent, meaning that more frequent doses of PDE5 inhibitors resulted in even lower risk.
Goyfman emphasized that this study was observational, and only found an association between PDE5 inhibitors and decreased mortality compared to patients taking alprostadil.

The Study Had Some Limitations
The researchers gathered data from 16,548 Swedish men who had experienced both ED and a heart condition in the last six months. Of those, under 2,000 received alprostadil and the rest received
The study was observational, meaning researchers did not control the experiment but rather gleaned trends from data.

Most of the evidence linking marijuana to heart attack and stroke is based on reports from people who smoked it. So its hard to separate the effects of cannabinoid compounds on the cardiovascular system from the hazards posed by the irritants and carcinogens contained in the smoke. Because cannabis smoke is known to cause airway inflammation, wheezing, and chest tightness, people with lung diseases should not smoke it. Other people who should just say no to marijuana include those who may be vulnerable to developing schizophrenia or addiction.

Black American Communities And Heart Attacks
While the overall risk of heart disease varies among different racial and ethnic groups in the United States, there is less data on heart attacks specifically.
According to the American Heart Association, the rate of cardiovascular disease in Black Americans is about 23 percent higher in men, and 33 percent higher in women, compared with the overall U.S. population.
Non-Hispanic Black Americans have the highest rate of death from heart disease at 208 per 100,000 people, according to the CDC. Next come non-Hispanic white Americans at 169, Hispanic Americans at 114, and Asian Americans at 86.
Researchers have found that among U.S. hospitals with sufficient data, the rate of death from heart attacks in Black patients was nearly identical to that of white patients. But the hospital readmission rate for Black patients was 4.3 percentage points higher than for white patients, according to an investigation published September 2018 in JAMA Open Network.

More Research Needed To Confirm Findings
We showed in 2017, in a paper in Heart, a longer life span for men with a recent MI who were taking PDE5i, said Holzmann in an interview with Healthline. There is one study on patients with diabetes with similar findings.
In addition, in the general population it has been found that men who use PDE5i have a lower risk of cardiovascular disease, he continued.
Goyfman said the reduced risk of heart attack might be due to how this class of drug influences one particular risk factor for cardiovascular disease.
PDE5i are known to cause a decrease in systemic blood pressure, he said. And since high blood pressure is a risk factor for cardiovascular disease, it is theoretically possible that patients with PDE5i would have lower, or better controlled, blood pressures.
Although he cautioned, this is strictly hypothetical, and can only be verified by conducting a randomized controlled trial.
Until that occurs, this study is unlikely to change current medical practice, Goyfman concluded.
