  • Gravitational disorderfor example, immobility
  • Congestive heart failure
  • Venous thrombosis or obstruction, varicose veins
  • Hypoproteinaemiafor example, nephrotic syndrome, liver disease
  • Lymphatic obstruction

Fatigue and lethargy

Fatigue and lethargy in chronic heart failure are, in part, related to abnormalities in skeletal muscle, with premature muscle lactate release, impaired muscle blood flow, deficient endothelial function, and abnormalities in skeletal muscle structure and function. Reduced cerebral blood flow, when accompanied by abnormal sleep patterns, may occasionally lead to somnolence and confusion in severe chronic heart failure.

Do Dogs Have Heart Attacks

In humans a heart attack usually refers to myocardial infarction . Myocardial infarction refers to death of the cells in an area of the heart muscle or myocardium. Cell death is usually due to oxygen deprivation caused by obstruction of the coronary blood vessels that supply blood to the heart muscles. Heart attacks are rare in dogs but unexpected and sudden death in dogs diagnosed with any form of heart disease is possible.

Moderate Stage Of Chf

At Stage C, as the heart continues to fail, the person develops weakness and significant fatigue, shortness of breath or palpitations with the slightest physical exertion. She begins to limit activity because of these symptoms as she is only comfortable when resting. A doctor’s evaluation and testing shows moderate signs of heart dysfunction,

Other possible symptoms might include more visible edema of the lower extremities and the hands might also swell as the fluid congestion in body tissues increases. Shoes and rings might be tighter. The pulse might become weaker because of the struggling heart.

Surgery For Heart Failure

Your doctor may recommend surgery to implant a medical device that helps the heart function more effectively:

  • Pacemaker, which maintains a steady heart beat in people with a slow or irregular heartbeat
  • Implantable cardioverter defibrillator , which monitors the heart for fast rhythm and delivers an electrical shock to reset normal rhythm
  • Left ventricular assist device , which takes over the pumping action of the heart

People with advanced heart failure may be candidates for heart transplantation. A heart transplant replaces the diseased heart with a donated heart from a person who has died.

Signs And Symptoms Of Congestive Heart Failure

Congestive Heart Failure

Following are the most common signs and symptoms of congestive heart failure. However, each child may experience symptoms differently. CHF signs and symptoms in children may include:

  • Failure to gain weight in infancy. Some babies will have normal length, but low weight for age
  • Shortness of breath or labored breathing
  • Needing to take frequent rest breaks while playing with friends
  • Falling asleep when feeding or becoming too tired to eat during infancy
  • Cough and congestion in the lungs

The severity of the condition and symptoms depends on whether and how much of the heart’s pumping capacity has been affected and/or how much extra blood is going to the lungs. The symptoms of CHF may resemble other conditions or medical problems. Always consult your child’s physician for a diagnosis.

Imaging And Other Tests

Other tests provide pictures of the heart and surrounding structures or show how well the heart is working:

  • Electrocardiogram : An electrocardiogram test uses small sensors to measure heart rate , rhythm and electrical impulses.
  • Chest X-ray: A chest X-ray is a picture of your lungs, heart and surrounding structures. It can show whether there is fluid in your lungs from heart failure, or if your heart muscle is enlarged.
  • Echocardiogram : Echocardiography uses sound waves to create images of your heart. It can show how thick the heart muscle has become, as well as measure ejection fraction.
  • MRI: MRI is an advanced imaging test that takes pictures of the heart and surrounding structures. It helps determine your heart function and size and whether there are any changes in the heart muscle. A cardiac MRI may help your doctors identify causes of heart failure.

Oedema

Swelling of ankles and feet is another common presenting feature, although there are numerous non-cardiac causes of this symptom. Right heart failure may manifest as oedema, right hypochondrial pain , abdominal swelling , loss of appetite, and, rarely, malabsorption . An increase in weight may be associated with fluid retention, although cardiac cachexia and weight loss are important markers of disease severity in some patients.

What Are The Four Stages Of Congestive Heart Failure

The New York Heart Association developed the four stages of congestive heart failure depending on the functional capabilities of the heart.

  • Class I: Patients will have no problem while performing physical activity.
  • Class II: Patients will have minor limitations of physical capacity due to a marked increase in physical activity. This leads to weakness, increased heart rate, difficulty breathing, and chest pain, but they may be comfortable at rest.
  • Class III: Patients with marked limitation of physical activity in which minimal ordinary activity results in weakness, increased heart rate, difficulty breathing, and chest pain however they are comfortable at rest.
  • Class IV: Patients cannot carry on any physical activity without discomfort and have symptoms of heart failure or chest pain, even at rest.

Congestive Heart Failure Drugs

What Is Congestive Heart Failure CHF: Everything You Need To Know

There are several medications that can be used to treat CHF, including ACE inhibitors, beta-blockers, and more.

ACE inhibitors

Angiotensin-converting enzyme inhibitors open up narrowed blood vessels to improve blood flow. Vasodilators are another option if you cant tolerate ACE inhibitors.

You may be prescribed one of the following:

voluntary recall of 5 lots of the drug Accupril due to the presence of nitrosamine. Nitrosamine, a known carcinogen with the potential to cause cancer, was found to exist in the drug at levels greater than the Acceptable Daily Intake as determined by the FDA. This recall is specific only to a handful of lot numbers and does not affect all Accupril tablets made by Pfizer. If you take Accupril tablets, talk with your pharmacist or doctor and they will help you determine if your medication has been impacted by the recall.

ACE inhibitors shouldnt be taken with the following medications without consulting a doctor, because they may cause an adverse reaction:

  • Potassium-sparing diuretics and potassium supplements. These diuretics can cause potassium buildup in the blood, which may lead to abnormal heart rhythms. Examples include: riamterene , eplerenone , and spironolactone .
  • Nonsteroidal anti-inflammatory drugs .NSAIDs such as ibuprofen, aspirin, and naproxen, can cause sodium and water retention. This may reduce the ACE inhibitors effect on your blood pressure.

Beta-blockers

This may be achieved with:

Diuretics

What Are The Internal Mechanisms That Occur On Congestive Heart Failure

Congestive heart failure may be a result of the physiological mechanisms taking place internally due to various reasons-

It can stem from a condition medically known as cardiomyopathy. In this, the heart muscles essentially start to weaken and the normal pumping action is done forcefully.

High blood pressure can damage the muscles of the heart making it weak and unable to pump blood efficiently.

Damage to the heart valves either due to leakage or even blockage may lead to improper functioning of the valves to carry blood throughout the body. In a typical problem of heart leakage, there may be back flow of the blood, making the heart pump blood harder to compensate. This pressurizes the heart and may lead to congestive heart failure.

An incident of heart attack, medically called the myocardial infarction may also lead to congestive heart disease. This may lead to damage of the blood vessels or the chambers of the heart thereby leading to congestive heart disease.

When To See A Healthcare Provider

Congestive Heart Failure: Causes And Symptoms

If you are experiencing chest pain, shortness of breath, sudden swelling of the arms or legs, or fatigue on exertion, you may want to get checked by a healthcare professional.

Heart failure and CHF are chronic, progressive conditions that can often be managed with timely treatment, so do not wait to be seen by a healthcare professional if you experience any of the aforementioned symptoms.

Precipitating Causes Of Heart Failure

  • Arrhythmias, especially atrial fibrillation
  • Angina pectoris or recurrent myocardial ischaemia
  • Iatrogenic causefor example, postoperative fluid replacement or administration of steroids or non-steroidal anti-inflammatory drugs
  • Poor drug compliance, especially in antihypertensive treatment
  • Thyroid disordersfor example, thyrotoxicosis
  • Pulmonary embolism

In a patient with appropriate symptoms and a number of physical signs, including a displaced apex beat, elevated venous pressure, oedema, and a third heart sound, the clinical diagnosis of heart failure may be made with some confidence. However, the clinical suspicion of heart failure should also be confirmed with objective investigations and the demonstration of cardiac dysfunction at rest. It is important to note that, in some patients, exercise-induced myocardial ischaemia may lead to a rise in ventricular filling pressures and a fall in cardiac output, leading to symptoms of heart failure during exertion.

Can Congestive Heart Failure Stem From Unhealthy Lifestyle Habits

Congestive heart failure can occur if a person lives an unhealthy lifestyle for prolonged period of time. Some of the conditions include-

Obesity: Lack of routine exercise and prominent obesity may put a pressure on the heart directly which may lead to heart failure.

Salty Food: Consumption of excessively salty food or high intake of salt in any form may facilitate water retention making the person prone to congestive heart failure.

Alcohol: Excessive consumption of alcohol and smoking affects the functions of the internal organs and may lead to congestive heart failure.

Medicines: If the patient does not take proper medication for existing conditions like high cholesterol or high blood pressure and does not confirm with the medical therapies, it can increase the risk of congestive heart failure.

Medications For Heart Failure

Certain medications can help treat heart failure and its underlying cause:

  • Aldosterone blockers block the effects of the hormone aldosterone, encouraging the body to release sodium and water. They also help prevent scarring in the heart.
  • Angiotensin receptor-neprilysin inhibitor : Treatment with sacubitril-valsartan reduces excess fluid in the body and relaxes blood vessels. This makes it easier for your heart to pump blood. Alternatives to ARNIs include angiotensin-converting enzyme inhibitors andangiotensin receptor blockers .
  • Beta blockers make the heart beat more slowly and with less force. They also help keep heart rhythm regular.
  • Sodium-glucose transport protein 2 inhibitors are diabetes drugs that have recently been found to be very beneficial in heart failure. They help remove glucose via the kidneys into the urine.

Other medications can relieve the heart failure symptoms:

  • Diuretics reduce the amount of fluid in the body.
  • Vasodilators dilate the blood vessels and reduce the hearts workload.
  • Digoxin helps the heart beat stronger with a more regular rhythm.
  • Anti-arrhythmics control arrhythmia and maintain normal heart rhythm.

Early Attention To Heart Disease

Pathophysiology of Congestive Heart Failure (CHF)

Congestive heart failure is one of the leading causes of death in the elderly and is not curable. The best course is to avoid or treat early known causes of the disease to prevent or limit damage to the heart. See your doctor if you notice early signs that might indicate CHF such as shortness of breath or fluid in your legs.

Does Heart Failure Show Up On Ecg

An electrocardiogram is often abnormal in patients with heart failure, although up to 10% of patients may have a normal ECG. Natriuretic peptides are a useful biomarker for heart failure and a negative result can rule out the diagnosis. This can be helpful in determining who should be referred for echocardiogram.

When Should I Get Emergency Care

Go to the ER or call 911 if you have:

  • New, unexplained, and severe chest pain that comes with shortness of breath, sweating, nausea, or weakness
  • Fast heart rate , especially if you are short of breath
  • Shortness of breath that doesn’t get better if you rest
  • Sudden weakness, or you can’t move your arms or legs
  • Sudden, severe headache
  • Fainting spells

Shortness Of Breath While Lying Flat And With Exertion

Shortness of breath while lying flat. People who have left heart failure may experience shortness of breath when lying flat . The person will start using two or three pillows to prop themselves up while in bed so that they can breathe easier. If the shortness of breath is more severe, the person may begin sleeping in a chair or recliner.

Shortness of breath with exertion. Shortness of breath with exertion is referred to as dyspnea on exertion. Dyspnea on exertion occurs because the lungs cannot supply enough oxygen and the heart cannot adequately pump oxygen-rich blood to the body. Initially, the shortness of breath may occur when walking long distances or up flights of stairs, but as the heart failure progresses, less activity may produce more symptoms.

Is Chf Due Mainly To Heart Valve Disease

CHF is most commonly caused by valvular insufficiency. It is estimated that 80% of the canine CHF cases are caused by MVI. However, there are many other causes. Disease of the heart muscle , irregularities of rhythm, and narrowing of some of the major blood vessels can also cause CHF. Initially, MVI results in left-sided congestive heart failure. If left untreated, the heart failure may progress to involve both sides of the heart.

Doctors Notes On Congestive Heart Failure

Congestive heart failure refers to a condition in which the heart is no longer able to pump blood sufficiently to meet all the bodys needs for oxygen. The inability of the heart to pump sufficiently ultimately causes fluid to leak from capillary blood vessels, causing many of the characteristic symptoms. Any disease that can weaken the heart can cause congestive heart failure. Some of the more common causes are:

Fluid buildup in the lungs is known as pulmonary edema, which leads to shortness of breath. Other associated symptoms include:

  • problems sleeping when reclining.

What Is the Treatment for Congestive Heart Failure ?

The treatment for congestive heart failure is to some extent dependent upon the cause of the heart failure and usually involves a combination of different medications, which may include:

  • Diuretics to control fluid buildup
  • Medications to control blood pressure, including angiotensin converting inhibitors , beta-blockers, androgen receptor-2 blockers, or others
  • Medication to strengthen the hearts activity such as digoxin
  • Vericiguat , which a type of drug known as an oral soluble guanylate cyclase stimulator
  • Nitrate medications to control chest pain
  • Blood-thinning medications to prevent clots

Some people, depending on the cause of their CHF, may require:

  • Surgery to repair or replace heart valves
  • Coronary bypass surgery
  • Insertion of a pacemaker, ventricular assist devices
  • Heart transplantation

Heart Failure With Preserved Ejection Fraction

Signs and symptoms of heart failure

In heart failure with preserved ejection fraction , formerly known as diastolic heart failure, the left side of the heart retains its ability to pump but the stiffening of the heart muscle does not allow it to relax during beats.

Losing the ability to relax during beats means that the left ventricle chamber loses its capacity to adequately fill with blood, so when it pumps, less blood is pushed out.
